Dog Adoption Application Form (Thailand)

As a volunteer organisation, SCAD does not receive any official funding; it exists on donations from the public, income from its Social Enterprises and funds/services provided by the SCAD founders and volunteers. If you are able to make a donation to SCAD when adopting a rescued pup or dog, it will be warmly appreciated and will go towards helping other rescued animals. (Cheques payable to “Homeless and Sick Dogs’ Funding Association”)

The dog/puppy that you adopt will have received age-appropriate vaccines. All puppies, as well as some adults, might require further vaccinations, fecal exams and possibly worming medications. If we have medical records, they will be given to you.

All puppies over 4 months of age will be sterilised prior to adoption. If you adopt a younger pup, appropriate steps must be taken to have them spayed/neutered as early as possible. We strongly recommend that all adopted dogs are kept on heartworm preventative medicine.

We cannot guarantee, nor can we be responsible for the health of the pets being adopted. Most of the pets under our foster care have come from unknown origins. Whilst all have been checked by a veterinarian, and every effort has been made to ensure the basic health of our dogs, some might be incubating diseases which are not readily apparent at the time of adoption. No monetary compensation will be given by SCAD in such cases.

Under no circumstances should a SCAD puppy be sold, traded, put out on the street, given to your maid or disposed of in any way. If you cannot continue to care for the dog/puppy, you should inform SCAD immediately.

Feed your dog well! We recommend Hill's Science Diet, specifically formulated for puppies, adults and seniors.
